EliHealth Access Program: Request for Elitone

We understand that not everyone can afford essential pelvic health care. That’s why we created EliHealth—a nonprofit dedicated to providing Elitone devices to those facing severe financial and medical hardship. Through our donation matching program, we refurbish and distribute a very limited number of donated devices to those in critical need.

To qualify, applicants must meet the following criteria:

• Demonstrate financial need, such as qualifying for Medicaid, overwhelming medical expenses, or serious health challenges preventing access to treatment. Note: You will be asked for proof.
• Experience urinary incontinence and seek access to effective treatment.

Due to high demand, only the most in-need applicants will receive a device, and others may be placed on a waitlist.  If you think you meet the requirements, please complete the application below.
